Improve provider section of resource page on web app #1094

  1. Add suggest button to providers section
  2. Add additional text and link to curation guidelines
  3. Update assignees on all issue templates (remove myself - ideally, this should let you specify a team but that doesn't actually work)
  4. Don't show providers box on resources with no own terms (e.g., afo). This is true even if a URI format exists (e.g., chiro)
