Fix to Issue 142 #163

This PR fixes Issue https://github.com/biothings/myvariant.info/issues/142. It introduces the following changes:

  1. Revision of the dbNSFP dumper/parser/mapping for release 4.3a
  2. Addition of the dbNSFP dumper design doc
