Install packages and extended inserts #146

Should we change to using extended inserts in the install packages by default?

The main pluses are speed, and avoiding 'max_questions' issues with some hosts. 
The main minuses are that some old hosts don't support extended inserts and 
that this might involve more work for Fried.

Any thoughts?

After I get the normal packages out of the door, I'll give this a go finally! I 
think I already have a cunning idea for this.
