Removal of xhtml theme #235

Browsers provide basic html5 feature sets for quite some time now. By removing 
xhtml theme we can focus on one theme. Rudimentary html5 support existed even 
as far back as ff 3.6 and ie6. Moving forward, we will set a minimum supported 
browser version of Chrome 3.0 (IGB browser equivalent). At the time of writing, 
current browser versions are:
Chrome: 24.0
Firefox: 18.0
Safari: 5.1.7
Opera: 12.10
IE: 10

Writers of edk themes will need to move to the html5 theme style or maintain 
html4/xhtml themes themselves by making parallel changes.

Now just need to delete from kb3_config where cfg_key = 'style_name' in a 
future db update. Consider also removing style_banner altogether.
