non-unique ID handling #44

Currently dealt with by appending the table name to the local identifier.  This 
is not sufficient for tables with multiple concepts, in which the IDs might 
only be unique to the column.

Fixed.  Both the table name and column name are now used to construct the new 
ID, which should be sufficient at the dataset level.
