SBS improvements pull request #2

These get the SBS output working again and add several improvements:

Added more messages corresponding to squitters 0,4,5 and 11.
Distinguish between surface and airborne extended squitters.
Have the SBS output flight ID tracking
Have the SBS record milliseconds
