Approval request is coming in multiple cards. #5023

Saw approval request in 3 cards back to back.

I’ve debugged it, and the approval request is not appearing in multiple notifications. There are multiple notifications for multiple requests. When a request is declined, it does get declined; however, the issue is that the declined request is not being removed from the relay. As a result, the declined notification continues to appear in the stack.
