Request PTO - sends an email to pto@bitovi.com with a link to approve or deny. On approval, this adds a calendar entry to the PTO calendar

What about the policy of how far out you need to ask (https://github.com/bitovi/bitovi/blob/master/great-people/keeping/benefits/paid-time-off.md#pto-policy)

1-week notice must be given for taking 4-8 hours off in a given week
2-week notice must be given for taking 2-4 days off in a given week
1-month notice must be given for taking 1-2 weeks off
2-month notice must be given for taking more than 2 weeks off

Maybe we don't restrict people requesting it, but it adds some information to the email sent to pto@bitovi.com.
