'Unknown node: issue' on inline issue roles #24

Sometime recently (probably during the fixes for 0.5.2 and not before), inline/subsequent issue roles - i.e. ones in the description and not at the start of a line - stopped getting translated correctly, leaving them around until render time, which as usual gets mad.

Have a fix already, need to write a regression test since clearly this wasn't getting tested correctly.

This might be a dupe of #22 but I need info from the submitter to verify this. If the bug was introduced during my recent changes, then his issue being the same one is unlikely and might be some unrelated problem w/ same symptom.

Yea there were no tests for this at all ಠ_ಠ wtf. adding one ASAP...

Also confirmed that #22 is distinct since the problem case in this ticket was fixed in the linked commit, but #22's problem remains. Will tackle it separately.
