Support for Oculus SDK v0.4.1 #21

This contains all the work needed for going from v0.2.5 to v0.4.1.

@dsjolie @janoc Any objection to merging the v0.4 dev branch to master?

I couldn't test it (as there is no Linux support in the SDK yet), but looks generally OK to me. I will perhaps submit fixes later if needed, once Oculus supports Linux with the new SDK again.
