MD5 fixes + progress + --extractdir #82

Here are some updates we had to make to modify the image of a OnePlus 6T A6013 (McLaren Edition)

Fix a bug with MD5 calculations due to files not being flushed before re-reading.
Speed up MD5 calculations by doing it as files are written instead of re-reading after.
Added a --extractdir option. Default is still "extract" directory, as before.
Added simple "Encrypting ..." / "Copying ..." to stdout to show progress of long operations.

Also this change which may be too annoying, we can revert if you don't think it's useful:

Assert attributes (sizes and offsets) are in sync while encrypting and copying.
