Updating `model_run.ipynb` and adding config file. #39

I've added a config file to specify the countries in our analyses, so that they do not need to be hand-specified in each file. I've also cleaned up the model_run.ipynb notebook to make it a bit more clear and updated paths so that all estimates end up in their own folder.
