This is something new when compared to the old gallery. The basic idea is to create an always visible link on all of the pages that allows the visitor to leave feedback on the pages.
[x] Create feedback form, it should collect: the current url, name, email, message items
[x] DB should also track whether the feedback was handled or not (simple date field, handled_at)
[x] Place the dialog open button in an absolute position
