Minor fix to reverse_proxy.sh #21

Not sure why X-Forwarded-For was mentioned twice. This PR will remove the duplicate entry and normalise the casing across all application-ids.

I was looking for some example configurations for crucible and nginx but only found examples with lower case for. As there is no official configuration from Atlassian. I have now found more examples even without header fields, so I think we can correct it.
