Registration form #4

Create registration form with 'Sign Up' button, that redirects from Home Page.
'Sign Up' button will be not displayed, when user is authorized.

Form will contain next fields and buttons:

Nickname (independent from server)
Email
Password (with confirmation)}

Another options like osu! OAuth can be available after registration & authorization.
