Add option to disable activate_first #20

Sometimes I don't want to have first node being activated (see old image). It would be nice to add new setting to deactivate first node activation (see new image).

I support SVG format in new sphinxcontrib-blockdiag (>= 1.1.0). You can use it with blockdiag_html_image_format option at conf.py.
