Certain extensions get duplicated and stuck in the list #5

Steps to repro

  1. Install Microsoft Markdown Notebook
  2. Filter any working query
  3. Filter markdown notebook
  4. Filter the original query again

Also note any extension without an image displays a broken link instead of a placeholder.

This bug should have been fixed, and I haven't encountered a similar situation during testing.
