Move PHX Tools into this repository and convert it to a monorepo managed by Lerna #250

In line with https://github.com/Asymmetrik/phx-tools/issues/62, we will be moving these packages together so everything can be managed a little easier.

Having everyone in one place will improve developer experience, issue tracking, and package quality. Read more about it here.
