Try to make IDs unique. #20

Cross-referenes are broken, though. But I figure it's a start toward resolving #19.

I've cherry-picked this into the unique-labels branch in https://github.com/briandfoy/text-multimarkdown/. This breaks the tests, so I'm not merging it yet.
