First and foremost, I would have made this opt-in, not opt-out.
If someone opted in, I'd ask further:
How often would you like to be reminded? Once a month? Randomly?
Every day?
Which of your colleagues would you like to receive messages from?
Would you prefer not to receive messages from former colleagues?
On days when you are reminded, would you like to be asked if you
could use a reminder first, rather than having one offered blindly
because an earlier version of you consented?
Should a computer be able to inadvertently prompt us to think about
something at all?
