Fix master build #121

Hi there.

This pull request fixes builds for the current master branch. The first commit is basically #119, but I also had issues that seem to be caused by the use of proc_macro_hack.

I think it is worth bumping the version number to v0.3 due to the removal of proc_macro_hack.

Also, I understand that the repository is not maintained anymore, so I'm not sure what's the correct way to get this set of change accepted, just let me know if there is something I can do to help.

Thanks, merged. Not sure I want to publish a new version, though, don't really want to encourage people to use it in its current state.
