Simplify, standardize and fix bugs:

fix problems with focussing files while choosing from panel (remove work-around that moves files into one group)
don't open files while previewing (only preview already opened files, and open file when really switching to the bookmark)
use snake_case in code and filenames
use UUIDs instead of integers
remove visibility modes (keep it simple: just show the bookmarks for the current project, starting with the bookmarks for the current file)

I noticed some problems with this branch when it comes to restoring bookmarks when opening the project. In fact, I've started to doubt the feasibility to create any good bookmark system in Sublime Text: it's likely that any attempt will run into unexpected problems related to associating bookmarks with pieces of text that at some point may be moved or deleted. Perhaps it's better to just use a todo-type of plugin, like others have proposed...
