Add Establish integration tests #360

Add the following tests

[x] Establishing a channel with 0 money does not work
[x] Establishing a channel with non-zero customer money works
[x] Establishing a channel with negative money does not work
[x] Establishing a channel with a name that matches an existing channel does not work

Right now, the implementation hard-codes refusal of channels with positive merchant balances, so I am leaving out those tests for now.
