pvh: load initrd from fw_cfg #18

Hi Paolo, Liam posted a new version of "[PATCH v3 0/4] QEMU changes to do PVH boot" that includes a patch that I wrote in order to support initrd. This PR contains a patch for qboot to support this feature.
