Plugin "spring clean" #52

Hi @boonebgorges, I realise this is a substantial PR - hope it's not too large to assess properly. Basically I've given the plugin a "spring clean" while I was testing it with CommentPress.

I haven't changed any functionality as such - just fixed what was most obviously in need of TLC and imposed some phpcs code style on the main plugin files. I've left the themes directory largely alone since the themes appear to be obsolete. Likewise the groupblog directory, apart from a couple of text-domain changes.
