Drop coverage ignores

• attenuate check thresholds to match actual coverage
• disable per-file check until we get number up ( tracked in #49 )

Decided to just rip the bandaid, and go with all sources, no exclusions.

So in summary: we're at 76% coverage, and that tracks better with the sensation of found latent bugs; now time to go to work ™
