Want To Add Warn date and Alert Date(as date description) with Bubble popups date


What is the expected output? What do you see instead?
Want to see the bubble popups start date and end date with date description 
like Warn Date and Alert Date.

What version of the product are you using? On what browser and what
operating system?

I have attached the two file one for current format of bubble popup and
2nd for the expected format of date with description

Please help me to get expected format.
