patch to include list of loaded modules in crash_report output #275

Here's a patch for crash_report.mm that lists the loaded modules at the end of 
the crash report. 
This is what a 'regular' crash log on Leopard also provides and it may be very 
useful when looking 
for 3rd party code being loaded into an application when troubleshooting 
crashes...
