[BUG] Ignored Misunderstood Phrases Re-Appear

Description When you classify a phrase as ignored, it still appears in the misunderstood module when a user types it in again. Moreover, when a phrase is part of a workflow response, it still appears as a misunderstood phrase.

Expected behaviour When a misunderstood phrase is marked as ignore it should never re-appear. This eliminates tautology.
