Adding custom course fields as COURSE_xxxx #238

I had to change this for a customer, so I decided to create a pull request and share this solution with everyone else.

Unfortunately, even though I updated the language files, changed the version and updated the Moodle database, the help text displayed when clicking the "?" button has still not changed.

it's conflited with branch develop, and i can't see the conflits (i don't know why)
