feat(mdx): add MDX grammar injection #138

This injects the mermaid grammar into MDX code blocks annotated with the mermaid language.

Thanks for the contribution, it looks good! Can you add one small addition? Can you update package.json and the changelog like this commit to trigger deploying a new version?
