Codebook, title, and subtitle are printing at the bottom of the codebook #52

Overview

I don't have a good explanation for why, but starting today (2024-02-15) I noticed the word "Codebook" that was previously printed at the top of the codebook document now prints at the bottom. Additionally, if there is a title or subtitle, that is printed at the bottom of the codebook as well.

After a little experimenting, it looks like changing the object name cb_shell on line 99 of codebook.R to rdocx fixes the problem.
