Make sure you flashed the correct image to your card and the card is in the right slot for the image you flashed. Also make sure your system is running 4.5. How are you flashing the cards?
I've tested (3) SCPH-101 systems and had no issues myself. Perhaps try the TonyHax freepsxboot image tonyhax_scph-101_v4.5_slot2.mcd and see if you still have issues.

I'm flashing it with the Unirom bootdisc CD (this one: https://github.com/JonathanDotCel/unirom8_bootdisc_and_firmware_for_ps1) because i don't have a PS2 or 3, and i'm flashing the 171bdcec that is compatible with the 4.5 A PS1 slim. I Burned it with ImgBurn at 4x speed, and i used the disc swap method.

I actually modified the .bin so i can put this exact same mcd, and i'm still having some issues. (at first i burned the .bin with no modifications that i downloaded from github and it gave me bad results.)
I'll try and download tonyhax again to see if it works, but it will cost me another CD to do it.

Say, are your memory cards official or third parties? If they are third parties, it is most likely that they have a non-standard format and powers up with 7.5 volts, which could affect the flashing of the image.

You may need to find another way to flash the card. Can you share a photo of the card you're using? Both External and Internal? You might also want to clean the contacts while you have it open.
On my SCPH-101 both 4.4 & 4.5 worked for me. The chart told me I had 4.5 bios but everything else reported I had (4.4) a> [4.5-slot2 FreePSXBoot 2.1] image did work. But if you go by the date, I think they just used this hardware for both PAL (4.5) & NTSC (4.4) possibly. Anyway all I know is the chart is weird if you follow the row over it shows 2 consoles in that area. UniROM2.1 says one thing & wikipedia says halfway something else.
I'm having issues when I'm trying to boot to the FreePSXBoot hack in both my original and cheap gamestop memory card. When I press the X button in the memory card menu, it simply doesn't boot. I've been doing this with the Swap disc method and Unirom8 Disc to flash my memory card. Am I doing something wrong? Let me know.
