Add toolbar button for dialog #26

Something like the top image on http://wordpress.org/extend/plugins/gist-github-shortcode/screenshots/ may help when the author wants to insert a more advanced shortcode.

There could be fields / checkboxes for each option we support.

A video of a more advanced dialog can be seen at http://wordpress.org/extend/plugins/simple-gist-embed/ although this goes about adding new gists, and has potential for problems when a user has hundreds of gists assigned to them.
