Rename Content-{Length,Type} → Patch-{Length,Type} #114

Here's a quick draft for issue #97, to explore renaming:

Content-Length → Patch-Length
Content-Type → Patch-Type

...within any patch inside of a Patches: N update.

We still aren't sure whether to change Content-Range → Patch-Range.

Note: The relevant commit is https://github.com/braid-org/braid-spec/pull/114/commits/d250ecd1640dfa964c813a798121f6fd4bf0a238. Ignore the others—this PR was accidentally combined with https://github.com/braid-org/braid-spec/pull/113.
