To do:

[ ] Explain that counter() is evaluated at an element's origin position in the DOM (related to Named Flows).
[ ] Support an optional third argument for counter(): first | start | last | first-except
[ ] Describe what happens with counter-set/counter-reset/counter-increment properties on elements with 'display: none' and their descendants.
[ ] Add more automatic counters besides 'page': 'volume', 'volume-pages', 'total-pages', 'total-volumes', ...
[ ] List predefined counter styles in current implementations (decimal, lower-alpha, lower-roman, upper-alpha and upper-roman) and possibly show their definitions.
[ ] The Liblouis implementation supports the additional counter style prefix-p. However this style could be dropped because counter(..., prefix-p) can be replaced by 'p' counter(..., decimal).
[ ] Explain differences with original @counter-style rule.
