Add new user annotation column #108

Story: #100

Description

User wants to add a new annotation column such as my notes.

1- update column table 2- Alter all backing databases 3- Column edit form should come with some default values 4- Column form autocomplete should filter system tables

I'm not sure if this is covered in this point or in #13 (Most of the fields...) or elsewhere, but columns should be free text by default but ideally with options for controlled vocabularies (e.g., for data modality).
