PR fixes #71. Scrutinizer bot doesn't like how I embedded html source code into ExampleNote.php + this is my first PR, so any guidance appreciated. Once corrected, example note fixes #78

I've fixed the HTML and pushed the code. However GH didn't close this PR. Thanks for your contribution!
