Fixed broken documentation hash anchor links #72

In the README is a table of content with internal links. Some of these are now broken because the headings have been renamed.

ie this link no longer works:

https://github.com/brendanheywood/moodle-local_cleanurls#step-2-apply-tiny-patches-to-core

and it should be:

https://github.com/brendanheywood/moodle-local_cleanurls#step-2-apply-tiny-patches-to-core-moodle-29-and-30-only

So need to do a review and make sure they all work. Would be nice to enforce this in travis but possibly in the too hard basket for now.
