Add better column and row index names for operator construction #21

Some of these are good and descriptive (e.g., slip_rate_column) but some are stil things like 3*i+2 and I'm never going remember what that is in a month!

I've made some changes where appropriate but not too many because some were less intelligible with longer and more explicit index names.
