Add (prominent) home icon to topic title (and/or) breadcrumb #129

Add (prominent) home icon to topic title (and/or) breadcrumb to indicate that the current topic is the "home" topic, i.e., the starting point in the topic map graph. Users sometimes rename the "home" topic so it is not always immediately apparent that they are "back at" the "home" topic.
