Add configuration support for ELASTICSEARCH_URL environment variable #3

Heroku apps will use the ELASTICSEARCH_URL environment variable to specify the location of their index. Native support for that would be awesome.

@outoftime — it may be worth noting here that ENV['ELASTICSEARCH_URL'] includes the index name as well. A typical value would look like http://index.bonsai.io/2x9mm3uhs0lrhd2lanpw
