ACCOUNT INFORMATION Re-enter Password - form not same size with Password form (design) Alternate Email - label cut in 3 (design) DLSU Email - add new form (optional)

PERSONAL INFORMATION Country - fixed City - fixed/no numerical Zip Code - numerical Address - form is too short Cellphone Number - accepts 12xxxxxxxx (should only accept 09xxxxxxxx)

Academic Information Level - Undergraduate / Graduate (debatable) College - Add form for college Degree Program - in sync with College form to filter the list Graduate? - disable fields according to what radiobutton is selected No - Term input limited to 1,2,3 No - AY input limited to 2 digits xx-xx Year Level - move rightwards (see picture)... users who click Yes will assume that it still needs to be selected

BUTTONS Proceed to Step 1 - don't make it mandatory that after you sign up, you immediately go to ordering process, have a homepage (ex. reminders, notes) for logged in users first Back - "Exit"?? Back - prompt user if he/she is sure about exiting the registration page

does each item represent a different defect? If yes, then you might want to separate them into different issues.

@brianehenyo Hazel is in charge of the back-end for this, cla is in charge of the front-end.
