[x] regenerate UIDs per year / participant (using years, names, affiliations)

An acceptable solution for dual (multiple) affiliations might be to mark them as "d" (or "m") and then provide the correct affiliations in manually-filled columns (multiple_name1, multiple_id1, etc.).

The data might not have cases of triple affiliations, but there are some cases in the AFSP data.
