Remove a variety of deprecated code paths for v2 #873

This PR removes virtually all the code paths previously marked deprecated and outputting warnings. In addition, it removes the legacy plugins functionality (aka bridgetown_plugins Bundler group) and requires that all Bridgetown sites use the Initializers config.
