Resolves #231

Refactors the Dashboard List page to use a modular column based approach, a la gnomAD or the variant lists
Adds the D flag to denote multiple associated diseases, and the I flag to denote multiple associated inheritance patterns to the dashboard list page
Clarifies text on Orphanet prevalence using a map
