Add a Pull Request Template #500

Is your feature request related to a problem? Please describe. The lack of a Pull Request Template disrupts the project.

Describe the solution you'd like At least add a checkbox for pre-commit because too many people don't run pre-commit.

I wouldn't go as far as saying that it's "disruptive", but it would certainly be a welcome addition.
