Add url generator #48

I think this could be pretty cool, has a lower barrier to entry for someone who's foreign to the idea of browserify(cdn), like how I used to be am.

Submit form will open a new window with the generated url.

One problem: The index.html is generated from the README and handlebar templates. This PR breaks that.

We should find a good way to fix this. I'll ponder a bit.

Oops. We can perhaps create the form using JavaScript?
