Shipments and Receipts with mismatched company, contact, address

What steps will reproduce the problem?
1. In Shipping, click Schedule a shipment
2. Select a Sender Company
3. Click the green plus for contact and/or address, and create a contact and/or 
address, but assign it to a company other than the Sender Company selected in 
step 1.

What is the expected output? What do you see instead?
It should not be possible to have a sender contact or sender address that 
belongs to a different company than the selected sender company.

To fix this issue:

1) Hide the green plus icons for contact and address until a company is 
selected first
2) When using the green plus icons to add a contact or address, in the modal 
dialog box, do not allow selecting a different company than the one already 
selected.

This fix should be applied for both the sender and recipient sections in 
shipments.

This problem also affects the "Sender Information" of Receipts. Please apply 
the same fix there as well.
