XhtmlHandler - no TagHandler for <br /> #16

There is no TagHandler added for the <br /> element, so the
WikiScannerContext is never called at onLineBreak() when the html contains
this element.  I added a handler in my copy of XhtmlHandler right after the
handler for <hr />, and it seems to fix the problem.
